There are two completely different things that both look like white smoke, and telling them apart takes about a minute.

Thin white vapour on a cold morning that clears within a minute or two and has no smell is condensation. It is water, it is normal, and every car does it. Cold exhaust pipes hold condensation overnight; the engine boils it off. In winter you will see it from every car at every set of lights.

Thick white smoke that persists once the engine is warm, smells sweet, and hangs in the air is coolant being burned. That is usually a head gasket, and it is a different conversation entirely.

Check two things now: the coolant level in the expansion tank, and whether it keeps dropping with no puddle underneath the car. Coolant that disappears without leaving a trace is going somewhere internal.

Four questions that settle it

Of those, the coolant level is the strongest single indicator. Coolant does not evaporate out of a sealed system. If the level keeps falling and there is no puddle, it is going into the engine or out of the exhaust.

The head gasket symptom cluster

No single symptom proves a head gasket. The cluster does. Look for several of these together:

  • Coolant loss with no external leak
  • Overheating, or a temperature gauge that swings up and down
  • The expansion tank pressurising quickly after start-up, or visibly bubbling with the engine running — combustion gas being pushed into the cooling system
  • Sweet-smelling white smoke that does not clear
  • A misfire on one cylinder, from coolant entering that cylinder
  • Emulsion through the oil — on the dipstick, not just under the cap
  • A heater that blows cold, because gas has displaced coolant in the matrix

Two or three of those together is a strong picture. One on its own frequently is not.

The mayonnaise question

This deserves its own answer, because it causes more unnecessary alarm than anything else on the subject.

A creamy, mayonnaise-like emulsion on the underside of the oil filler cap alone is usually not a head gasket. On a car doing short journeys in winter, it is condensation collecting in the coldest, least-ventilated part of the engine. It is extremely common and on its own it means very little.

What matters is whether the same emulsion appears on the dipstick and through the oil itself. Coolant in the sump turns the whole charge milky, not just the cap. If the dipstick is clean amber oil and only the cap is creamy, take a breath — though it is still worth noting, because it tells you the engine is not getting hot enough for long enough, which brings engine oil sludge forward.

The test that actually settles it

Symptoms build a case; a test closes it. There are three worth knowing about.

The combustion leak test, sometimes called a block test, is the quickest. A chemical fluid is held in the neck of the expansion tank and air from above the coolant is drawn through it. If exhaust gases are present in the cooling system the fluid changes colour. It takes minutes, it is inexpensive, and a positive result is conclusive.

One caveat worth knowing: a negative result does not completely rule it out. A small breach may only open when the head expands at full temperature, so a test at idle can miss it. If the symptoms are strong and the test is negative, it is worth repeating hot.

A cooling system pressure test pressurises the system cold and watches for loss. It finds external leaks — a weeping hose, a radiator seam, a heater matrix — which is important, because those cause coolant loss without any head gasket involvement at all.

A cylinder leak-down test pressurises each cylinder in turn. Bubbles appearing in the expansion tank tell you exactly which cylinder is breaching into the water jacket.

Other things that produce white smoke

  • A cracked cylinder head or block — same symptoms, worse outcome, and only found once the head is off
  • A leaking inlet manifold gasket on engines where coolant passes through the manifold
  • A leaking heater matrix — but this smokes and mists inside the car, with a greasy film on the windscreen that will not wipe clear, rather than out of the exhaust
  • A leaking or badly timed diesel injector, which gives whitish smoke that smells strongly of unburnt diesel rather than sweet. Different smell, different problem
  • Automatic transmission fluid, on cars where a failed transmission cooler inside the radiator lets ATF be drawn in — usually a distinctive greyish-white

What to do

Do not keep topping up and driving on. Coolant entering a cylinder washes the oil film off the bore, and in a bad case enough can collect while the engine is off to hydraulically lock it — which bends connecting rods on start-up. That turns a head gasket into a bottom-end rebuild.

Be careful with sealer products. As a get-you-by on a low-value car about to be sold or scrapped, they occasionally work. They can also block the radiator and the heater matrix, and they make a proper repair messier and more expensive. They are a deferral, not a repair, and worth going into with your eyes open.

The first row is the one that matters. Testing first is what stops a weeping hose being quoted as a head gasket, and stops a head gasket being topped up for six months until it becomes an engine.

When should you call a garage?

Stop driving and call if the smoke is thick and persistent, if the temperature gauge is rising, if the expansion tank is bubbling, or if the oil on the dipstick is milky.

No action needed if it is thin vapour on a cold morning that clears as the engine warms, with a stable coolant level. That is just water.

Worth testing if the coolant level is falling and you cannot find a leak, even with no smoke yet. Coolant does not evaporate, and finding out where it goes is a cheap question with an expensive alternative.
